Forms to Bring with You
Every guest, participant, client, volunteer and visitor to Project Horse must have a signed Release Waiver and Medical Consent form on file with us. Please complete and bring these required forms on your first visit.
For the safety of all, we kindly request that you review and sign the Project Horse release waiver before your first visit.
In the unlikely event of a situation that requires medical treatment, Project Horse will need to have emergency contacts on file and consent for emergency medical treatment procedures.

Client Services Agreement
This agreement is for those starting individual or family services therapy or experiential learning services, and spells out guidelines, expectations and responsibilities for all parties involved, including Project Horse.
Release of Information
In order for Project Horse’s treatment staff to speak with anyone else about you (or your child’s) therapy, experiential learning or coaching services, you must provide written permission. A Release, signed by you, must be in place for every professional or practitioner you permit us to speak or coordinate with.
